Liability Insurance for Vendors

Download the Certificate of Liability Insurance Requirements for Road Service Vendors.

Please ask your insurance provider to issue an Acord 25/Certificate of Liability Insurance detailing the following coverages and then forward a copy of the certificate to Chassis Support Services, LLC via e-mail at Vendorinsurance@rapidlinkrepairs.com or fax to (704) 626-3340. The certificate must be dated within thirty (30) days of submission.

Auto Liability:

Vendor Party performing Services, are required to procure and maintain, at their sole expense, a commercial automobile insurance policy with a combined single limit of $1,000,000 or greater, of which no portion can be self-insured.
Auto policy must be an “ANY AUTO” or SCHEDULED AND HIRED” policy. If the policy includes “SCHEDULED” autos, please provide a schedule for review.

General Liability:

Vendor Party performing Services, are required to procure and maintain, at their sole expense, a commercial general liability insurance policy with a combined single limit of $1,000,000 per occurrence or greater, of which no portion can be self-insured.

Worker’s Compensation:

Vendors engaged in Road Service, are required to maintain and procure worker’s compensation insurance covering applicable statutory benefits for applicable state(s) where Services are performed.

Certificate Requirements:

Additional Insured
Chassis Support Services, LLC must be named an Additional Insured on the automobile and general liability policies. Please ask the certificate producer to state the Additional Insured language as follows:
“Chassis Support Services, LLC has been named an Additional Insured with respect to the auto and general liability policies”

Certificate Holder

Certificate Holder/Notify Party shall read as follows:

Notice of Cancellation/Modification/Material Alteration

Chassis Support Services, LLC shall be given thirty (30) day notice prior to any cancellation, modification, or material alteration of the policy, including but not limited to, a reduction in coverage, decreased policy limits and/or removal of additional insured. This notice must be provided to Chassis Support Services, LLC. via certified mail.

Waiver of Subrogation

A waiver of subrogation must be granted in favor of Chassis Support Services, LLC, its subsidiaries, affiliates, insurers, agents, and employees for all policies. (Please ensure that the waiver language is clearly stated in the Operations Box on the Certificate of Insurance and that the appropriate boxes have been marked.)
